Jemma’s Story
Jemma was referred to Shaping Outcomes in 2023 after her son received an NDIS plan. He was assigned a Key Worker and supported through to receiving a diagnosis, from there all the supports that he required were engaged. Once he started primary school, the wheels began to fall off, Shaping Outcomes advocated with Jemma to keep him at the same school with his friends. Through this process the Key Worker supported the school to apply for more support funding in the school to assist with meeting his needs. He loved attending Lego Club at Shaping Outcomes where he worked on his social skills.

Team Lemonade
Nathans story began before Shaping Outcomes and is still continuing today. His mum tells how Shaping Outcomes was an integral part of his journey and hers. It has set her up not only to support her son but to branch into opening a support service for teens and adults with disabilities. If you’re lucky you may see Nathan perform as he is quite the singer and for many years was front man for ‘Us the Band’.
